The form is available in Portable Document Format (PDF) format. In order to view PDF documents, users must first download the free Acrobat Reader software. Detailed instructions for downloading and installing the Acrobat Reader are available at www.adobe.com/acrobat/readstep.html. For best results, please make sure you have installed version 6.0 or later of the Acrobat Reader. Before applying for visa, please ensure that your passport is valid for at least six months from the date of your application.
